About MIST

Milestone Pharmaceuticals Inc. (Nasdaq: MIST) is an emerging commercial-stage biopharmaceutical company advancing innovative cardiovascular medicines to benefit people living with certain heart conditions. Milestone’s lead product is CARDAMYST® (etripamil) nasal spray, a novel calcium channel blocker, which is FDA-approved for the conversion of acute symptomatic episodes of paroxysmal supraventricular tachycardia (PSVT) to sinus rhythm in adults. Etripamil is also in Phase 3 development for the control of symptomatic episodic attacks associated with AFib-RVR. https://milestonepharma.com/

$MIST Almost half of PSVT patients are 65+ and eligible for Medicare. We can check the 2027 Medicare formularies within a few days, and some early signals make me expect a better than par result. Some Medicare employer plan payers (SilverScript and Humana) have disclosed that Cardamyst will be covered in 2027. Several downstream EGWPs that adopt those formularies have also published their 2027 drug lists based on the upstream formulary. So far, plans such as Delaware, CalPERS, and the NC retiree Medicare plan have decided to cover Cardamyst in 2027 under favorable conditions, and there are many other EGWPs that used the same formulary IDs as those plans in 2026. They are highly likely to adopt the same 2027 formularies and cover Cardamyst as well. Yes, all of the clues I mentioned are from employer plans, which are different from individual Medicare plans. Nevertheless, it's a positive signal that the company has taken a good step forward in gaining access to the Medicare system.

$MIST I'm going to test my reverse psychology theory. This POS is ALWAYS red. There's no hope. It's going to RS then go OTC. Let's all get out while we still can!

$MIST another patient post, worked within minutes with only one spray in one nostril. It will be the standard of care-just a matter of time.

With a tiny-cap stock like $MIST and such a thin order book, the whales cannot build meaningful positions in advance without distorting the price. Institutional funds can meaningfully step in after confirmed good news hits and volume expands. Until then, a lot of potential buyers are structurally unable to participate in size, and we have to endure all the bear taunts, shorting, and the fear of not knowing where the bottom is. I don't like the rules of this game, but it's the destiny of a tiny one-product biotech. Ultimately, MIST has to prove itself through TRx. Although we are behind expectations now, the positive build-up continues and favorable signals keep accumulating. I believe the reward in the end could be much larger than the market currently implies. Thank you, friends, hope a little good luck comes our way :)

$MIST Anyone here that thinks this is going to zero is a daft brush. We may hit .59 but i dont see it going below that. If it does it will be a gift and afterwards a violent uptick will happen. PT is 1.80-2.20. Paytience is needed. No emotions in trading. I have been in this stonk since 2018. Made a lot of bank on this. Juice is worth the squeeze. This and $VSTM are very volatile and money making.

$MIST Another successful CARDAMYST experience posted today, kicked in within 7mins, helping the patient avoid an ER visit. I have strong conviction in this drug. If the product delivers, word spread, sales will eventually follow.

I am quite confident that $MIST’s Chinese partner, Everest Medicines, can sell Cardamyst very well in China. Cardiovascular disease is not Everest’s core commercial area so far. But Everest has a very close relationship with Hasten through CBC Group, a major/controlling shareholder of Everest and Hasten. Hasten has already established a robust cardiovascular commercial network in China by acquiring Takeda’s existing cardiovascular brands and sales team in 2022. In September, the two companies significantly expanded their cooperation, and Everest will distribute Hasten’s products in China. This means Everest is effectively gaining access to Hasten’s existing cardiovascular commercial infrastructure and sales know-how, which should be valuable for launching Cardamyst in the Chinese market. It looks like shareholders of both Everest and Hasten do not like this cooperation, but a stronger cardiovascular sales platform at Everest is clearly a positive development for MIST shareholders.

$MIST - H.C. Wainwright analyst maintained $8 price target on 9/22/2026. We also have 2 (5) start analyst from TD Cowen and Wells Fargo that have an $8.00 and $7.00 price target as of August 13th. Let's hope they are more right than wrong. Keep in mind the revenue forecast is $1.14 Million to 1.17 Million. And we know that we already have $2 Million in the bag with the China revenue plus we had $1.034 Million in deferred revenue on the balance sheet. Maybe we see $1.5 Million in product revenue for Q3. It will be interesting to see if they have sold any product to Everest Medicines as an initial build for the China inventory, or if they can shed some light on how the China ramp is going.

$MIST unbelievable. Who would have thought that we’d see these levels again. I still think it has huge upside potential in the long run

$MIST - Today will be the 17th straight trading day that falls below the $1.00 listing requirement. I know that this is concerning to many of the posters on this board. There are several reasons to believe Q3 reporting is expected on November 12th. The current analyst revenue projections are averaged at $1.14 Million to $1.17 Million with the lowest projection of $800,000 and a high projection of $2 Million. What do we know ahead of the report. Milestone showed $1.034 million in deferred revenue on Q2 report if all of that is realized in Q3 than the company should report around $1.5 million in regular revenue. We also know that there will be a $2 Million milestone payment from Everest Medicines for China Approval so now we are looking more like $3.5 Million in Revenue excluding and pre inventory build for China roll out. Then there is the European approval in early 2027 coming up and Milestone needs to get a distribution partner in place.

$MIST I bought some more. It’s 10th day below $1. But I do think this is more biotech sector related selling pressure than the individual stock of $MILESTONES. We should have moved up due to the good trial news from Canada. So I don’t blame the Company totally rather hope Joe learns to play the market a little better in PR release. You have to boost and market yourself if the street not recognizing you and your stock. Don’t get lazy and take for granted that somehow just because you have a FDA approved drug then all is good. I also agree that that top executives and Board members should buy 50-100 K shares in the open unless they have none public material news Btw during my buying in late hour in between $0.79-0.78 ranges I felt the MM was not really dumping per se because my BID were at 3000 -5000 shares per order and they didn’t dump on me but spread out in 100/200/300/500 multiple executions. Anyway the Marco market & record SI will play into day to day fluctuations. BL

$MIST I think what a lot of retail fails to factor in appropriately, is that cardamist is designed to treat a condition that is both episodic and chronic. The episodic nature will impact when/how often it is prescribed proof of effectiveness and the speed of uptake. In turn, this necessarily dictates the ramp up pace and why it doesn't reflect the aggressive $$ modelling for a much higher post approval sp. So, I think we need to watch for at least another 2-3 quarters. It's a slow grind I know, but as long as the script trajectory continues upward, the company will establish a foothold and make money. For investors, the ramp up has to outpace cash burn and that will lead to additional income to extend the window for P3 Afib trials, reduce dilution potential and lead more quickly to a potential buy out. If the trajectory flatlines, then it's a whole other story. I am still counting on a buy out down the road. Adulting is hard.

$MIST forced fund selling under $1 last couple of days creates a unique opportunity. Cash on hand still $170 million Coverage doubled at the very end of last earnings. So you saw only 25% coverage on 16 weeks commercial launch. Fantastic position here. Gift

Interesting MIST angle after Novo’s Capital Markets Day Novo explicitly outlined a “disciplined bolt-on business development strategy” and wants to diversify its pipeline beyond diabetes/obesity, including cardiovascular disease. That’s potentially relevant to $MIST: FDA-approved CARDAMYST, China approval already secured, Europe pending, and a Phase 3 AFib-RVR program underway. Novo’s materials do not mention MIST specifically, so this is not evidence of a deal. But the strategic fit is worth watching—especially given MIST’s tiny valuation relative to Novo.
