$MREO could have a very interesting Q4. Yesterday at Cantor, management said newer setrusumab analyses show fracture reductions in additional bone types and in higher-fracture patients. FDA discussions are now focused on whether there is a viable regulatory path forward, with clarity expected by year-end. My rough scenarios: • Full new P3 required: ~$0.25–0.50 • Smaller targeted study: ~$0.60–1.00 • Existing + extension data provide a BLA path: ~$1.50–2.50+ • Add Sentynl exercising the alvelestat option: $2–3+ becomes plausible At ~$0.27 today, a credible setrusumab path could completely change the $MREO valuation.

$MREO On Setrusumab, they do not want to give updates until a final feedback/plan is available. This is Q4. They have a holistic data set showing & biological activity and impact of the drug, and they know what happened in the arms leading to endpoint miss. FDA still wants fracture analysis on specific fractures, and Mereo/Ultragenyx have Post-hoc, non-disclosed data showing impact of Setrusumab on Patients who experienced fractures upfront (there was a very bad mix over the trial arms). Most patients switched to OLE study, both from Setrusumab, placebo and biphosphonate. 12 month OLE data will be available in October. Exchange with FDA will come first, then with other agencies. They did not indicate more, as they aim to share the final decision/path only.

Optimization of endpoints is basically based on Atalanta trial data, which showed positive impact on Patients under augmentation on exacerbstions. The new endpoints design is intended to show diiferentiation towards soc augmentation, giving another usp. besides oral treatment —> Better for pricing and competition

Is this simply positioning ahead of Sentynl/Alvelestat and Setrusumab, or does the market already know something we don’t? Any concrete information behind the move?

$MREO MREO — Q4 2026 could be decisive Following management’s latest comments, the next 3–4 months could be particularly important for MREO, with two independent catalysts developing in parallel. 1️⃣ ALVELESTAT — Sentynl / Zydus Sentynl is working with MREO on the development strategy and discussions with the FDA regarding the Phase 3 design. Phase 3 is expected to start in early 2027 if Sentynl exercises its option. However, Alvelestat could impact the share price before 2027 through three potential events: option exercise, favourable FDA feedback, and confirmation of a Sentynl-funded Phase 3. The agreement provides for up to $40M in upfront/R&D payments, up to $435M in potential milestones, plus tiered double-digit royalties on US sales.
